Critical Care Ultrasound

Point-of-Care Ultrasound in Critical Care

Point-of-care ultrasound (POCUS) gives appropriately trained critical care clinicians real-time bedside imaging that can complement physical examination and support focused cardiac, pulmonary, vascular, abdominal and hemodynamic assessment in critically ill patients.

In intensive care environments, portable ultrasound can be particularly valuable when clinical conditions change rapidly and repeated bedside reassessment is required.

Common Critical Care POCUS Applications

  • Focused cardiac and hemodynamic assessment
  • Lung and pleural ultrasound
  • Shock and targeted volume assessment
  • Vascular access and procedural guidance
  • Selected abdominal and renal assessment
  • Serial bedside reassessment

Why Point-of-Care Ultrasound in Critical Care Matters

Point-of-care ultrasound in critical care is a focused form of bedside ultrasound performed and interpreted by the treating clinician to answer specific clinical questions. Unlike comprehensive diagnostic ultrasound examinations, POCUS can be integrated directly into bedside assessment and repeated as the patient's condition evolves.

Current critical care ultrasound guidance addresses applications including septic shock, acute dyspnea or respiratory failure, targeted volume management and cardiogenic shock. Appropriate training, competency and clinical integration remain essential.

Cardiac POCUS

Cardiac & Hemodynamic Assessment

Focused Cardiac Assessment

Focused cardiac ultrasound may support rapid bedside assessment of cardiac activity, ventricular function, chamber size and selected causes of hemodynamic instability.

Pericardial Effusion

Subcostal and other focused cardiac views can help appropriately trained clinicians identify pericardial fluid in patients with hypotension, shock or cardiopulmonary deterioration.

Shock Assessment

POCUS can complement clinical assessment when evaluating possible contributors to shock, including cardiac dysfunction, altered volume status and selected pulmonary or obstructive processes.

Serial Reassessment

Bedside ultrasound can be repeated as clinical status changes, allowing ultrasound findings to be integrated with vital signs, laboratory results and response to treatment.

Pulmonary & Pleural Ultrasound

Lung Ultrasound in Critical Care

Lung and pleural ultrasound can support focused bedside evaluation of respiratory deterioration. Depending on the clinical question, POCUS may help assess lung sliding, pleural fluid, B-lines, consolidation and selected sonographic signs associated with pneumothorax or pulmonary edema.

Pleural Effusion

Ultrasound can help identify and localize pleural fluid and support bedside procedural planning when intervention is clinically indicated.

Acute Dyspnea & Respiratory Failure

POCUS can complement clinical examination, laboratory testing and other imaging when evaluating potential pulmonary or cardiac causes of acute respiratory deterioration.

Hemodynamic POCUS

Shock & Targeted Volume Assessment

Hemodynamic assessment in critical care usually requires integration of multiple clinical findings rather than reliance on a single measurement.

Point-of-care ultrasound in critical care can provide additional bedside information concerning cardiac function, pulmonary findings, venous assessment and other selected indicators relevant to hemodynamic management.

POCUS Does Not Stand Alone

Critical care ultrasound should be interpreted together with clinical examination, hemodynamic information, laboratory data, treatment response and other diagnostic imaging when indicated.

Abdominal POCUS

Abdominal, Hepatic & Renal Assessment

Focused abdominal ultrasound may support selected bedside questions involving the liver, kidneys, bladder, urinary tract, free fluid and other abdominal findings.

Hepatic Assessment

Portable ultrasound can support focused visualization of liver morphology, selected hepatic abnormalities and abdominal fluid within an appropriate clinical workflow.

Renal & Urinary Assessment

POCUS may support focused assessment of the kidneys and bladder, including selected evaluation for hydronephrosis or urinary retention.

Ultrasound-Guided Procedures

Procedural Guidance at the Bedside

Vascular Access

Real-time ultrasound can help visualize vascular anatomy and support needle guidance during central and peripheral vascular access when performed by appropriately trained clinicians.

Pleural Procedures

Ultrasound can help localize pleural fluid and support selected pleural procedures according to clinician competency and institutional protocols.

Frequently Asked Questions

Critical Care POCUS FAQ

What is critical care ultrasound?

Critical care ultrasound is focused point-of-care ultrasonography performed and interpreted by appropriately trained clinicians caring for critically ill patients.

How is POCUS used in the ICU?

ICU POCUS may support focused cardiac assessment, lung and pleural ultrasound, shock evaluation, targeted volume assessment, vascular access, selected abdominal imaging and serial bedside reassessment.

Can handheld ultrasound be used in critical care?

Yes. Handheld ultrasound can support selected critical care POCUS applications when the device is suitable for the examination and the clinician has appropriate training and competency.

Which Sono Mobile® ultrasound is suitable for critical care?

Sono Mobile® CL64 provides convex and linear scanning with a phased-array preset and supports multiple cardiac, pulmonary, vascular and abdominal POCUS applications.

Does POCUS replace formal diagnostic imaging?

No. POCUS is a focused bedside examination. Comprehensive echocardiography, diagnostic ultrasound, CT or other imaging should be obtained when clinically required.
